As you know, Southeast Queens is facing unprecedented challenges with the COVID-19 pandemic and we need you to help check on the most vulnerable members of our community. The SEQ Wellness Ambassador program has been put in place to make sure our seniors are staying well, embracing social distancing, and taking care of themselves. To support this, volunteers will make weekly check-in calls to the seniors throughout Southeast Queens.
Each volunteer Wellness Ambassador will be assigned to a team leader and given 10 to 20 seniors to call each week;
Each senior will receive an introduction call from the volunteer Wellness Ambassador informing them that their senior center requested us to check in on them each week;
If the Senior would like verification, the volunteer wellness ambassador will give the senior a name and number of a contact at the senior center.
For each check-in call, the volunteer Wellness Ambassador will have a script to follow including a wellness check form to complete.
Other resources – If seniors are requesting assistance, the volunteer wellness ambassador will log the specific need, instruct the senior to call their respective senior center for help, and then the team leader will follow up with the center to ensure the senior receives assistance.
Meals – If the Senior is requesting meals delivered, the volunteer Wellness Ambassador will give the name, address, and phone number to their team leader.
